Aloha Activation Network Associate Organization Application

A recognized partner with Aloha Activation Network (AAN).

Requirements: Proof of the following:

1. Adhere to all federal and state tax and employment laws and regulations. 

2. Provide your own liability insurance.

3. Display AlohaActivationNetwork.com with the logo on your website.

4. Read, understand, and approve the AAN mission: AlohaActivationNetwork.com

5. Sponsored by an AAN officer

6. Sign the Contract/Agreement

Please put "Associate Organization Application" in the email subject and include the following:

1. Name: Your full name as filed with the IRS.

2. Doing Business names (DBA), if any.

3. Your logo and business name that will be placed on our website.

4. Mailing address: to show proof of residency in Hawaii.

5. Email address: contact info.

6. Text phone number: cell number for quick info/contact.

7. In the rest of the email, please include

   1. A brief statement of what you believe the mission of AAN is.

   2. Executive officer sponsor: Dr. Gene Behl 

   3. Any questions you might have.

   4. Before typing your name at the bottom of the email, write the following statement: "I accept the terms and acknowledge that all the information provided above is true and accurate to the best of my knowledge."
