- Doing Business names (DBA)
- Your logo to be next to your name on our website
- Mailing address: to show proof of residency
- Email address: contact info
- Text phone number: cell number for quick info/contact
- In the body of the email please include
- A brief statement of what you believe the mission of AAM is
- Executive officer sponsor: _______________________. If you need a sponsor please let us know
- Any questions you might have.
- At the bottom of the email before typing your name write the following statement: "I accept the terms and acknowledge that all the information provided above is true and accurate to the best of my knowledge."
